C9, C9/B, G/F#, G
REMEMBER: guitars are tuned down half a step. 99% of the people forget that.

I figured out a way to play the outro in standard tuning. It sounds ok, and it beats having to tune down a 1/2 step.
E----x----x----1----2
B----7----7----1----2
G----6----6----2----3
D----4----4----3----4
A----x----x----3----4
E----7----6----1----2
Just listen to the cd for actual timing. For the 1st chord, use your third finger on the low E string and your second finger on the G string, then switch them for the 2nd chord. That way you don't have to move your first or third finger. Also, focus more on the GBE strings for the F and F# chords. It's not a perfect replica to what Dave does live, but i think it sounds pretty good.
